Research question

Do keratinized mucosa width, mucosal thickness or supracrestal tissue height independently predict later peri-implant disease or marginal bone-level changes?

Study design

The review followed PRISMA 2020, SWiM, QUIPS and an adapted GRADE framework. Twenty-seven reports representing approximately 28 potentially unique cohorts were included. Primary synthesis prioritized adjusted, temporally valid and clustering-aware estimates; meta-analysis was secondary and exploratory.

Main findings

  • Adjusted evidence was sparse, heterogeneous and often inconsistent; no single soft-tissue dimension showed consistent independent prognostic value.
  • An exploratory analysis of adequate/wide versus narrow keratinized mucosa and peri-implantitis produced RR 0.54 (95% CI 0.33-0.87), but the prediction interval crossed no effect.
  • Other pooled comparisons for mucosal thickness, marginal bone loss and supracrestal tissue height were imprecise or highly heterogeneous.
  • All eight principal exposure-outcome evidence bodies were rated very low certainty, with high risk of bias and limited independent replication.

Interpretation

Adequate keratinized mucosa may still support hygiene, comfort and recession control. Current evidence is insufficient to claim that a single phenotype dimension independently predicts peri-implant disease, and early bone remodelling should not be treated as a surrogate for peri-implantitis.
